The case study must include the age category or specific lifespan. Ensure you include information relating to the individual’s holistic health (physical, mental, spiritual, emotional) and important demographics, social determinants of health, and culture. You must include at minimum two social determinants of health applicable to your client. You will be applying the knowledge you developed to two major concepts we covered in class. This should come from your readings, lectures, and classroom discussions. It is imperative to include subjective and objective data that answer some of the “assessment” parts of these concepts. You are required to find at least one research article to support your social determinants of health or the concepts you addressed in your case study.

Using the concept map template, we have been using in this course, you will complete the concept map using the case study you created. You must include subjective and objective data to support your nursing diagnosis. Your concept map should have three nursing diagnoses. One of each of the following types of nursing diagnosis:

• Actual Nursing Diagnosis
