Case Study:

Collette is a 60-year-old lady. Collette lives with her partner,Max.Collette has two young adult children,Mary and Max,who both still live at home whilst studying full-time at university. 

 

Collette works in reception at a law firm,Monday to Friday 8:00am to 4:00pm.However,recently one of her co-workers resigned and occasionally works until 6:00pm,depending on client jobs.She spends most of her shifts sitting at a computer.During the work week most of her physical activity is walking from the carpark to her desk, and to and from the printer. 

 

Earlier this year Collette was looking at reducing to part-time as Max had received a pay raise the previous year. Unfortunately, due to a workplace injury, Max is on workers compensation leave and they are unable to afford mortgage repayments and support their adult children, so she has remained full- time.Max being unable to work has cause a lot of additional stress at home.Prior to Max's injury,Collette and he would go for daily afternoon walks with their dog, but this has ceased in the past 6 months.Collette and he would go for daily afternoon walks with their dog, but this has ceased in the past 6 months.

Collette had quit smoking for the past 10 years but has recently started smoking again in the last 6 months. In both of Collette's pregnancies she was identified as having gestational diabetes. However, she received very little education about how to manage this at the time. Two years ago, Collette's mother passed away. Collette and her mother were very close. Collette's mother had T2DM. Collette has no other pre-existing medical conditions and takes no regular medications. She has been through menopause and no longer menstruates.


Collette's key clinical measurements are as follows: 

Height: 165cm
Weight: 72kg
Waist circumference: 81cm
Hip circumference: 93cm
Blood pressure: 121/82mmHg


Two weeks ago, Collette visited her GP with complaints of fatigue, which she thought was due to her current work and family stressors. However, upon investigation, her GP identified through a routine blood test that her fasting glucose was slightly elevated. He sent her for further testing, an Oral Glucose Tolerance Test, and reported that she meets the "pre-diabetes" parameters. When her GP told her the news, she broke down in tears. Her results were as follows:

Fasting Glucose: 6.8mmol/L and 2-hour OGTT: 8.5mmol/L.

Collette is very motivated to make changes, however, she struggles with time restrictions and having to care for her partner.
