She is a widow, currently living by herself in her own home. Judy has always worked part-time as a primary school teacher but due to her mobility issues has now retired. She does not own a car and relies on public transport. She lives 10km from the closest health care facility. Judy has two grown-up children, who live close by, and who assist when they can. They will be present for the first visit.
The referral is from the local Aboriginal Medical Service (AMS). The request for daily dressing was from the Aboriginal Liaison Officer (ALO) for the community nurses to attend home visits for daily dressings to the leg ulcer as AMS is not able to attend to this need. The ALO is unable to come to the first home visit for an introduction.
The accompanying discharge letter from the hospital states that Judy may be eligible to be registered for Closing the Gap, and a doctor has now registered her for this service.
