Mixed Dementia

George is 85-year-old Non indigenous man. He admitted Acute care in Mercy care on 16/08/2019 and he was diagnosed as Mixed Dementia, NIDDM, Bowel cancer, HTN, Cataracts, Alcoholism, Vitamin deficiency and Osteoporosis.  

He underwent colorectal surgery 2012 and later on metastasis 2013 and he refused  further treatment.

George lives alone with his own private residence. He is allergic to sulphur, Elastoplast, Bee stings. His nutritional status is Diabetic diet.

GP prescribed following medication  Ardixgliclazide, Axit, calcium carbonate, celapram, Diaformin, Paracetamol, Aspirin and Vitamin D and tablets has been given  by crushed and mixed with yogurt.
