Case Study

Bio/geographical data

Mrs Wihongi is an 81-year-old Māori lady living with her three daughters and seven grandchildren in a village surrounding the Marae in a semi-rural township. Her daughter would like her mother to go to the hospital for respite care because of her worsening health conditions, but Mrs Wihongi does not want to go there.

History of presenting problem

Mrs Wihongi’s GP has asked you to review her in her home because of the frequent visits they have made over the last couple of weeks and the deterioration in her condition. Mrs Wihongi has been coughing for the last 5 days with yellow sputum. Mrs Wihongiis getting more and more short of breath (SOB) on exertion; she only managesto walk from her bed to the bathroom. The oedema around her legs has become worse. Daughter found her reduced appetite and does not want to get out of bed. She is tired but alert.

Task Details

  1. Read the case study above.

  2. Provide a brief overview of the patient case and relevant assessment findings as it applies to the nursing assessment framework. This is not about repeating the case history, you will need to demonstrate how each piece of data is correlated to the Te Whare Tapa Whā framework.

  3. You are then required to critique and analyse the selected patient case, interpreting and analysing all relevant assessment findings. This will include relevant health history, physical examination findings, psychosocial and health risk.

  4. Based on a single chronic condition that this patient has, demonstrate an in-depth understanding of the pathophysiological processes associated with the patient’s condition.

  5. Then you will develop a comprehensive therapeutic plan of care with a clear rationale from your findings in the assessment making sure you plan care relating to each of the domains of the nursing framework provided. The plan must include psychosocial and spiritual influences as well as the importance of whanau and whakawhanaungatanga for the patient. Your plan will also include pharmacological and non-pharmacological treatments, patient education and nursing recommendations supported by current literature and best practice evidence.

  6. You will demonstrate collaborative practice with referrals to appropriate interprofessional follow-up and support services. Plans will show evidence of partnership with the patient, whanau (family).

  7. Traditional Māori health acknowledges the link between the mind, the spirit, the human connection with whānau, and the physical world in a way that is seamless and uncontrived. The nursing assessment framework chosen for this case study is Te Whare Tapa Whā.
