Task Instructions

1. Take a large piece of paper and draw a fishbone diagram (see below).

2. Write a problem statement at the head of the fish that is specific to your circumstances e.g. “I did not achieve the grade that I has set for myself as my personal goal”, “I could not spend enough time on my studies to do as well as I wanted to.”, “I was satisfied with my grade, but I felt a bit stressed and under pressure”. You are advised to personalise your problem statement to a problems that you experienced. 3. The aim is now to explore branches of the fishbone to do a deeply considered root cause analysis of your problem. Recommended branches include the 5 W’s - What, Where, Who, Why, When - however you may choose bones of your choice like: skills, systems, people, place, policies, methods, materials and/or technology. Bear in mind that your word count limits how many branches you can fully explore so it is recommended that you have no more than 5 branches. You need to focus on areas that were within your control for this to be a meaningful exercise for positive change / kaizen.

4. Now either take pencil or sticky notes and jot down the problems you may have faced in each branch or bone that help you back from achieve what you wanted to achieve. Asking yourself “Why did the [branch name] affect or cause my problem?”

5. Once you have completed this activity look for: a. Are there any connections and links between items? b. Can you identify any patterns? c. How did these connections, links and patterns contribute to the outcome you achieved? d. What will you do differently in your next subject?

6. Now you can start to write your report
