This assessment task is divided into two parts: Part A : Lifestyle Journal and Part B Reflective

Part A : Lifestyle Journal (no word count)

To successfully complete Part A of this assessment task, you will create a detailed seven-day lifestyle journal based on John Arden’s SEEDS wellbeing framework. Your journal will document activities related to the five SEEDS elements: Social Connections, Exercise, Education, Diet and Sleep. You will also identify activities that enhance or impair your wellbeing and include a mood rating for each day.

5) Sleep

  • Record your sleep patterns, including duration and quality of sleep.
  • Reflect on how you feel when you wake up.

6) Health-Enhancing Activities

  • Note activities that promoted health and wellbeing.

7) Health-Impairing Activities

  • Note activities that diminished health and wellbeing.

8) Mood Rating

  • Record your mood rating for the day (score between 1:10, with 1 being a very low mood and 10 feeling elated).

Part B : Reflective Report (1,000 words ± 10%)

Instructions:

In Part B of Assessment 2, you will write a 1,000-word reflective report (± 10%) focusing on ONE (1) of the SEEDS elements documented in your lifestyle journal. This reflection will involve exploring your experience with your chosen element (Social Connectivity, Exercise, Education, Diet or Sleep),

identifying the barriers and enablers you encountered, and proposing strategies to overcome any challenges.

Throughout your report, you should incorporate relevant guidelines and/or supportive literature that emphasise the importance of your chosen SEEDS element for wellbeing. Additionally, you are required to integrate your experience into one of the following behavioural change models within your report: the transtheoretical model or the health belief model.

Please use the following format to structure your report

1) Introduction (100 words)

  1. a) Introduce the SEEDS framework and specify the element you will reflect on (Social connectivity, Exercise, Education, Diet OR Sleep).

  2. c) Discuss TWO (2) potential strategies for overcoming the barriers you identified and why these will be beneficial in transforming this health behaviour.

5) Theoretical Integration of SEEDS Element (200 words)

Integrate your experience with the SEEDS element using ONE (1) of the following behavioural change models: the transtheoretical model or the health belief model.
