• What are you trying to achieve?
• How much do you want to achieve?
• When do you want to achieve it by?
• Who will benefit from it?
•  Provide a justification for the outcome measures chosen and explain how these will help to understand if a change is an improvement.
• Consider how the triple or quadruple aim will be addressed.
