Based on the information provided in Appendix A, build a database in MS Access. Create a name for your database. The database should include a minimum of 4 tables (Patient, Contacts, Signs & Symptoms, and Physicians) and should contain fields that collect data to fulfill the goals and business rules outlined in Appendix A. Enter data into each table (see Appendix B for examples). Create the required relationships between the tables. Use the relationships window and ensure that all fields are visible in the diagram and save the relationship report. Include a design template for each table that will be used to create the table in the database, including any restrictions placed on field properties to ensure data entry would be correct and consisten.
Goals/Objectives
The goal of the database is to provide a user-friendly interface for physicians and clinicians who will be collecting and entering data on the Variola outbreak and spread, and on the mutation to variant SP-63. The database will allow for tracking of patients, disease progress, contacts of those infected, and physicians. The database tables will be supported by forms, queries, and reports. Security will be implemented to protect the privacy of the patients (we will not really implement this, only research how it is done.
