For this assignment, you will review a case study and a completed Checklist of Community Mobility Skills which was provided by your OTR. This is a new patient for you, and the OTR has identified some concerns with their ability to remain mobile in the community based on the checklist results. Case Study:
Lilian is 91 years old, a widow, and lives alone. She still drives for community mobility, and has recently obtained a renewal on her driver’s license. Lilian values her independence, and does not like to take community mobility or public transportation. Lilian has 1 daughter who lives two hours away, and comes to visit her once a month, when possible. She has experienced recent health problems as follows: COPD, NIDDM, Cataracts, arthritis in R hip, lymphedema in B Les, and asthma. Due to her COPD dx, she requires 2 lpm when at rest and 4 lpm when actively performing tasks. She does not like that she has an 02 tank with her all day, and frequently trips over the tank cord when performing functional mobility. Lilian displays overall Fair endurance and requires rest after 6 minutes of activity or standing.
While Lilian knows her neighborhood well, she loathes the use of technology in the modern world and feels that cell phones and other devices are “complicated”. Her daughter has provided her with a mobile phone, but the battery frequently dies, as Lilian does not use it and forgets to charge the device. She is comfortable with her landline phone and is able to use paper maps when her daughter marks her destinations ahead of time. The paper maps have become difficult for her, however, due to her cataracts diagnosis. Lilian frequently gets lost when traveling outside of her immediate 2-block radius, and cannot contact her daughter or others due to limited knowledge of how to use a mobile phone. When someone attempts to help her from the community, she is often unable to provide her current or intended location due to her becoming overwhelmed. She also becomes confused when attempting to travel to her frequent MD appointments, as dates and times are very perplexing to her.
Based on Lilian’s identified areas of concern, list 3 OT standardized assessments that you could administer to her over the course of your treatment to assess function and track progress. You should consider all OT assessments you’ve been exposed to throughout the program, utilize the list of program standardized assessments that is present in the module, and provide a detailed justification for your choices.
