1. The purpose of this Project is to assess the skills and knowledge required to work in partnership with older people and their carers to implement interventions in the context of an individualised plan to reduce risk.

2. Your Trainer and Assessor will inform you of when you are required to submit the Project. This assessment should take you approximately four (4) hours to complete.

3. Your Project will be performed by completing the following tasks:

A. Provide a report that documents the current issues surrounding care for older people.

Your report should include:
 

• Legal and ethical considerations for aged care, including duty of care, work role boundaries and documentation requirements

• The current trends of care needs for older people, e.g. the need for flexibility in care arrangements

• Policies that have been developed in aged care

• Major risk areas and lifestyle factors of older people

• How older people are treated by the care system and the tensions that may exist

• between an older person and an organisation assisting with care needs

• Working with the older person and carer in the course of your work Community Services

• The need for risk minimisation strategies and how standardised risk assessment tools may help.

B. Perform a simulated assessment of risk, including identifying risk minimisation strategies, how these would be implemented and evaluated.

C. Perform a risk assessment which also identifies risk minimisation strategies for the care of an older person.

• For this you should:

• Implement the strategies determined

• Monitor the strategies

Evaluate how these are working, after a suitable period of time (determined by the assessor/trainer).

4. Your Project will be performed in the following context:

•  As this is all theoretical, there is no need to be undertaken in the workplace or simulated work environment.

5. Your Project must address the following key elements:

• Assist with the preparation for a risk assessment

• Contribute to the identification of risks

• Implement risk minimisation strategies

• Monitor risk minimisation strategies

6. You will require the following resources, materials and/or equipment to complete this Proje
