It’s your last night shift before two weeks off on leave and you have had a busy night already. It’s only 10pm, you’ve done 5 jobs, and another 9 hours to go till shift ends. You are now called to a caravan park on the Hawkesbury River for a patient with shortness of breath. 

Your response time is 25 minutes and you are around 40 minutes to the nearest appropriate hospital (travel time).

Finding the caravan was a bit tricky but eventually you are introduced to Charlie who is a man in his 70’s. He is unable to talk to you and his daughter tells you the lead up to the events of today. Whilst doing your primary survey you note tachypnoea and tachycardia immediately. 

They have been out on the river water skiing with family today, Charlie had complained of feeling unwell for most of the day. Finally after a big day of activities he went to lay down and his daughter has just checked up on him to find him very short of breath. He has also been coughing quite a lot. 

A quick check of his pale skin makes you realise he appears obviously ill, is agitated, and has cyanosis around the face.
